If the Hipaa Worksheets or InForm templates are distorted when you open them, go to Control Panel > Internet Options > Security > Internet > Custom Level > ActiveX controls and plug-ins. Make sure this setting is enabled. Seems to only affect versions of Internet Explorer before 11.413.15063.0. The update version 11.0.43 seems to have fixed this.
Another way to test is to launch Internet Explorer and go to File > Open and select the .idf4 worksheet by viewing all files to open. If Internet Explorer displays a prompt for allowing ActiveX, then check for that setting.
