When accessing AuthAnvil Two-Factor Auth via Internet Explorer 9/10/11 you may notice multiple Token Information tabs. By disabling Compatibility Mode for Intranet sites will resolve this.
Tools > Compatibility Mode > Un-Check Display intranet sites in Compatibility mode.
Next simply refresh the page. You will see the page correctly displayed
